What you will do

  • Provide senior clinical leadership, specialist mental health assessment, and direct care across the Adult CMHT pathway.
  • Undertake comprehensive assessments, short-term interventions, ongoing care coordination, complex case management, and discharge planning.
  • Support complex formulation, personalised care planning, risk assessment, risk management, and safeguarding processes.
  • Implement and promote the use of DIALOG+ and other recovery-focused interventions.
  • Manage an agreed caseload, delivering assessment, formulation, care coordination, and profession-specific interventions.
  • Provide line management, supervision, leadership, and clinical support to Band 6 practitioners and wider team members.
  • Promote a culture of shared multidisciplinary responsibility, therapeutic engagement, and outcome-focused practice.
  • Collaborate with service users, carers, partner agencies, and healthcare professionals to deliver high-quality mental health support.
  • Maintain accurate and timely clinical records in line with professional and organisational standards.

Requirements

  • Professional Registration: Current registration with NMC, HCPC, or another relevant professional body.
  • Mental Health Experience: Significant experience working within adult mental health services.
  • Community Experience: Previous experience within Community Mental Health Teams is highly desirable.
  • Leadership Skills: Experience supervising, mentoring, or supporting junior practitioners.
  • Risk Management Expertise: Strong skills in assessment, safeguarding, care planning, and complex case management.
  • Communication Skills: Excellent interpersonal and multidisciplinary working abilities.
