Band 6 Community Psychiatric Nurse/Mental Health Practitioner required for an exciting locum opportunity within the Community Transformation Project in Manchester. This ongoing role offers a competitive rate of £26.00 per hour and the flexibility of both full-time and part-time working arrangements. Core hours are Monday to Friday, 9am-5pm, with flexibility available around start and finish times to support a healthy work-life balance.

Join a forward-thinking team working across Greater Manchester, including Manchester, Salford, Trafford, Bolton, and Wigan. We are seeking approximately 20 to 25 Community Psychiatric Nurses and Mental Health Practitioners to support Adult Community Mental Health Teams as part of this innovative transformation programme.

What you will do

  • Collaborate with multidisciplinary teams to provide comprehensive mental health care and support for adults within the community.
  • Conduct assessments and develop personalised care plans tailored to individual service users' needs.
  • Deliver therapeutic interventions and support in line with agreed care plans and recovery goals.
  • Maintain accurate, comprehensive, and timely clinical records and documentation.
  • Liaise effectively with healthcare professionals, partner agencies, carers, and support services to ensure coordinated care.
  • Monitor service users' progress and adapt interventions as required to support positive outcomes.
  • Promote recovery-focused practice and support individuals in achieving greater independence and wellbeing.
  • Contribute to the ongoing development and improvement of Community Mental Health Services.
