What you will do:

- Conduct comprehensive assessments to understand the needs and strengths of children with disabilities and their families.
- Develop, implement, and review care plans that ensure the provision of appropriate services and support.
- Work collaboratively with families, carers, and other professionals to safeguard and promote the welfare of children.
- Provide expert advice and support to individuals regarding matters of the Mental Capacity Act and Deprivation of Liberty Safeguards.
- Attend and contribute to court proceedings as needed.
- Maintain accurate and up-to-date records, adhering to all relevant legislation and best practices.
- Facilitate and support transitions into adult services where necessary.
