What you will do:

• Undertake comprehensive assessments of adults within the community
• Develop and implement person-centred care and support plans

Requirements:

• Experience in Adult Social Care is essential
• Strong assessment and care planning skills
• Minimum of 3 years’ references covering employment history

• Relevant Social Work qualification and registration (e.g., Social Work England)
