Social Worker – Adult Services (Older People)
Location: Caerphilly
Pay Rate: £30.00 per hour PAYE or £39.58 per hour Umbrella
Contract: Temporary
Contract Duration: 12 weeks
Hours: 37 hours per week

Seven Resourcing is currently recruiting for an experienced Social Worker to join an Adult Services team supporting Older People in Rhymney on a temporary basis.

This role is covering maternity leave and offers an excellent opportunity for a qualified social worker with experience in adult services, assessment, and care management to support vulnerable adults to maintain independence, dignity, and wellbeing.

The successful candidate will undertake assessments, care planning, and reviews, working within a multidisciplinary environment to deliver high-quality social care interventions.

Key Responsibilities:

• Undertake strength-based assessments of adults with care and support needs, with a focus on older people.
• Develop, implement, and review care and support plans in line with statutory requirements.
• Carry out risk assessments and ensure safeguarding concerns are identified and acted upon appropriately.
• Manage a caseload of service users, ensuring timely and accurate recording of all interventions.
• Work collaboratively with health professionals, voluntary organisations, and other partner agencies.
• Participate in multidisciplinary meetings and contribute to joint decision-making.
• Promote independence, wellbeing, and choice for service users.
• Ensure all practice complies with relevant legislation, policies, and professional standards.
• Maintain accurate and up-to-date case records and documentation.

Essential Requirements:

• Degree or equivalent qualification in Social Work.
• Registration with Social Work England or relevant UK social work regulatory body.
• Experience working within Adult Services, ideally with Older People.
• Strong knowledge of relevant adult social care legislation, including the Care Act 2014.
• Experience in assessment, care planning, and case management.
• Strong communication, analytical, and report-writing skills.
• Ability to work effectively within a multidisciplinary team.
• Enhanced DBS check required.
