What you will do:
- Perform strengths-based assessments and create personalised reablement plans for diverse caseloads.
- Act as an expert advisor on equipment use and decision-making strategies to support client independence.
- Conduct home and hospital visits, using your valid full UK driver's licence, offering compassionate, needs-driven service.
- Collaborate with multidisciplinary teams to ensure clients receive comprehensive and coordinated care.
- Maintain accurate client records, documenting assessments, reablement plans, and outcomes.

Requirements:
- Degree or equivalent in Occupational Therapy.
- Minimum of two years of post-qualification experience as an Occupational Therapist in the UK.
- Recent NHS experience is preferred.
- Registered with the Health and Care Professions Council HCPC.
