What you will do:

  • Work across all pathways of Intermediate Care including reablement, rapid response, and rehab beds.
  • Collaborate effectively within a multidisciplinary team to deliver comprehensive care and support.
  • Engage in the development and implementation of personalised rehabilitation plans for clients.
  • Offer expert guidance and support to enhance client independence and quality of life.
  • Conduct assessments and reviews of service users' needs and abilities.
  • Ensure a holistic approach to treatment that incorporates both community-specific and general needs.

Qualifications:

  • It is preferable to have experience working in multidisciplinary teams and rehabilitation.
  • Some post-graduate experience will be beneficial due to the complexity of our team's caseload at times.
  • A background in community work is also essential for this role.

Requirements:

  • Applicants must be registered with the HCPC.
  • A valid driving licence is required as you may need to travel between different work sites.
