What you will do:
- Serve as a duty practitioner, conducting thorough initial assessments and providing insightful risk reviews and management strategies.
- Develop and implement case formulations to tailor individualised care plans.
- Manage a varied caseload with care and precision, ensuring the best outcomes for each young person.
- Undertake safeguarding responsibilities, ensuring all practices are compliant with current legislation and guidelines.
- Engage in community-based work essential for this role, connecting with families and young people in their environments.
- Participate actively in multi-disciplinary team meetings and liaise with services including schools, children's social care, and VCSE partners to align on care strategies.
- Attend meetings such as child protection and Team Around the Child (TAC) to advocate for the welfare of the individuals in your care.
