What you will do:

- Provide comprehensive support to clients with learning or physical disabilities, long-term conditions, and those transitioning into adulthood.
- Conduct assessments and develop care plans to help clients maximise their independence and safety within the community.
- Engage with young carers, helping to support their continued care role in a sustainable manner.
- Work collaboratively within a multidisciplinary team to address issues such as dementia, substance misuse, homelessness, and hoarding.
- Utilise your driving skills to perform essential field visits as required.
