What you will do
- Conduct initial and comprehensive assessments of health and social care needs, either independently or with colleagues, and provide feedback to the Multi-Disciplinary Team.
- Act as a Care Coordinator under the CPA process, managing the overall care of an identified group of service users, focusing on recovery and social inclusion.
- Offer supervision to a group of staff and students on placement with the team, fostering their development and growth.
- Participate in MDT activities, including being on duty and clinic rota as appropriate to your professional expertise.
- Lead on primary care mental health liaison and link working as part of the Integrated Community Mental Health Team.
Essential Requirements
- NMC Registration: Current NMC registration.
- Driving Licence: A valid driving licence is required as part of the role involves community visits.
