What you will do:

• Deliver expert clinical pharmacy services across diverse inpatient and outpatient specialties.
• Evaluate prescriptions to ensure medicines are safe, clinically appropriate, and cost-effective.
• Conduct medicines reconciliation and promote safe prescribing practices.
• Provide specialised pharmaceutical insights to consultants, doctors, nurses, and other healthcare professionals.
• Monitor patients' medication therapy and offer evidence-based recommendations to enhance treatment.
• Engage in multidisciplinary ward rounds and contribute to comprehensive patient care planning.
• Assist in the development and implementation of clinical pharmacy policies, procedures, and best practices.
• Mentor and support junior pharmacy staff, fostering a culture of collaboration and development.
• Ensure adherence to all professional, regulatory, and hospital governance standards.
• Maintain accurate clinical records and contribute to quality improvement initiatives.

Requirements:

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Substantial experience working as a Clinical Pharmacist within a hospital setting.
• Strong clinical knowledge and excellent communication skills.
• Capacity to work independently and collaboratively within a multidisciplinary team.
• Experience in private healthcare is desirable but not essential.
