Senior Clinical Pharmacist – Reading, UK £37.77 Hourly Locum | Full-Time | Monday to Friday | 8:00am – 4:00pm | 15th June 2026 – 31st August 2026 (12 Weeks)

What you will do:
- Deliver expert clinical pharmacy services across diverse inpatient and outpatient specialties.
- Evaluate prescriptions to ensure medicines are safe, clinically appropriate, and cost-effective.
- Conduct medicines reconciliation and promote safe prescribing practices.
- Provide specialised pharmaceutical insights to consultants, doctors, nurses, and other healthcare professionals.
- Monitor patients' medication therapy and offer evidence-based recommendations to enhance treatment.
- Engage in multidisciplinary ward rounds and contribute to comprehensive patient care planning.
- Assist in the development and implementation of clinical pharmacy policies, procedures, and best practices.
- Mentor and support junior pharmacy staff, fostering a culture of collaboration and development.
- Ensure adherence to all professional, regulatory, and hospital governance standards.
- Maintain accurate clinical records and contribute to quality improvement initiatives.
Requirements:
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Substantial experience working as a Clinical Pharmacist within a hospital setting.
- Strong clinical knowledge and excellent communication skills.
- Capacity to work independently and collaboratively within a multidisciplinary team.
- Experience in private healthcare is desirable but not essential.
