What you will do
- Team Leadership: Lead and manage a team within the Community Living service, ensuring effective delivery of care.
- Care Planning: Conduct comprehensive assessments and develop evidence-based care plans.
- Clinical Oversight: Monitor patients’ conditions and escalate concerns appropriately.
- Staff Support: Guide, mentor, and support team members in delivering care programmes.
