What you will do:
- Hold a caseload of service users, conducting comprehensive assessments and creating tailored recovery plans
- Deliver evidence-based interventions, including brief therapy, psychosocial support, and relapse prevention strategies
- Offer harm reduction advice, covering safer usage and blood-borne virus awareness
- Collaborate effectively within a multidisciplinary team comprising clinicians, social workers, and peer mentors
- Engage with external partners such as GP practices, housing providers, and criminal justice agencies
- Maintain accurate and up-to-date case records in line with CGL policies, CQC standards, and data protection requirements
Person Specification:
- NVQ Level 3 in Health and Social Care or equivalent, or willingness to work towards it
- Desirable training in substance misuse, mental health, or safeguarding
- Understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks
- Familiarity with CQC requirements and clinical governance in community services
- Enhanced DBS check is mandatory
- Experience in substance misuse, mental health, homelessness, or equivalent support services
- Experience managing caseloads and delivering structured interventions
- Experience working with vulnerable adults facing complex issues including dual diagnosis and social exclusion
