What you will do:

• Hold a caseload of service users, conducting comprehensive assessments and creating tailored recovery plans
• Deliver evidence-based interventions, including brief therapy, psychosocial support, and relapse prevention strategies
• Offer harm reduction advice, covering safer usage and blood-borne virus awareness
• Collaborate effectively within a multidisciplinary team comprising clinicians, social workers, and peer mentors
• Engage with external partners such as GP practices, housing providers, and criminal justice agencies
• Maintain accurate and up-to-date case records in line with CGL policies, CQC standards, and data protection requirements

Person Specification:

• NVQ Level 3 in Health and Social Care or equivalent, or willingness to work towards it
• Desirable training in substance misuse, mental health, or safeguarding
• Understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks
• Familiarity with CQC requirements and clinical governance in community services
• Enhanced DBS check is mandatory
• Experience in substance misuse, mental health, homelessness, or equivalent support services
• Experience managing caseloads and delivering structured interventions
• Experience working with vulnerable adults facing complex issues including dual diagnosis and social exclusion
