This is an exciting opportunity for a seasoned Adult Social Worker to join the Hospital Discharge Team in Newcastle. In this temporary role, you will help facilitate the smooth transition of adults from hospital care to their homes, ensuring they receive the most suitable support at the right time. Course-correct transitions and your carefully crafted interventions during the initial 4-10 weeks post-discharge can pave the path for an enhanced quality of life, offering a significant chance to witness impactful results.

What you will do:
- Conduct strengths-based assessments to gather insights post-hospital discharge.
- Formulate and review comprehensive support plans that nurture independence and focus on optimal outcomes.
- Undertake Mental Capacity Assessments where necessary, ensuring client needs are met effectively.
- Engage in safeguarding enquiries and proactive risk management.
- Play a key role in the Continuing Healthcare processes, working closely with health professionals, families and the wider support network.
- Offer evidence-based recommendations geared towards promoting independence and enhancing safety.
- Diligently maintain accurate records and ensure completion of all required documentation.
About You:
As a dedicated Adult Social Worker, you will bring:
- A recognised social work qualification and registration with Social Work England.
- Experience in adult social care, underpinned by strong knowledge of safeguarding and strengths-based practice.
- The ability to navigate complex situations and exercise sound professional judgement.
- Excellent communication skills and the confidence to work collaboratively with partner agencies.
