What you will do:
- Conduct thorough assessments of service users to understand their needs, risks, and recovery goals.
- Develop, implement, and routinely review personalised recovery and support plans tailored to each individual.
- Engage with service users through one-to-one keywork sessions and deliver recovery-focused interventions to guide them towards their goals.
- Provide informative harm reduction advice, including guidance on overdose prevention and promoting safer substance use practices.
- Assist service users in accessing necessary treatment, healthcare, housing, education, training, and employment services.
- Regularly monitor progress, manage your caseloads efficiently, and maintain accurate and confidential case records.
- Proactively identify and address any safeguarding concerns and risks that may arise during your work.
- Collaborate with healthcare professionals, social services, criminal justice agencies, and community organisations to ensure a comprehensive support network is in place for each service user.
- Promote and encourage active engagement with recovery programmes and peer support networks, fostering a supportive community atmosphere.
