What you will do:
- Provide high-quality physiotherapy services across all pathways of Intermediate Care including reablement, rapid response, and rehab beds.
- Collaborate effectively within a multidisciplinary team to deliver comprehensive care plans.
- Adapt therapeutic interventions to suit the complex needs of your patients.
- Conduct thorough assessments and evaluations to establish and monitor individual care plans.
- Facilitate and manage treatments emphasizing rehabilitation and patient independence.
Requirements:
- Hold HCPC registration.
- Be an experienced car driver for patient visits and community interaction.
- Have some community experience and preferably, prior exposure to working in a multidisciplinary team and rehabilitation environments.
