What you will do:

• Provide high-quality physiotherapy services across all pathways of Intermediate Care including reablement, rapid response, and rehab beds.
• Collaborate effectively within a multidisciplinary team to deliver comprehensive care plans.
• Adapt therapeutic interventions to suit the complex needs of your patients.
• Conduct thorough assessments and evaluations to establish and monitor individual care plans.
• Facilitate and manage treatments emphasizing rehabilitation and patient independence.

Requirements:

• Hold HCPC registration.
• Be an experienced car driver for patient visits and community interaction.
• Have some community experience and preferably, prior exposure to working in a multidisciplinary team and rehabilitation environments.
