What you will do

• Care Act Assessments: Conduct thorough assessments to determine eligibility and support needs.
• Placement Coordination: Arrange placements and manage direct payments effectively.
• Court Work: Undertake responsibilities related to Court of Protection and Deprivation of Liberty Safeguards (DoLS).
• Mental Health Support: Deliver person-centred interventions for adults with mental health needs.
• Team Collaboration: Work closely with colleagues and partner agencies to provide holistic support.
• Record Keeping: Maintain accurate and compliant case records in line with statutory requirements.

Qualifications / Requirements

• Qualified Social Worker with active Social Work England registration.
• Proven experience in Adults Social Work, specifically within mental health services.
• Strong knowledge of Care Act assessments and Court of Protection / DoLS processes.
• Experience managing placements and direct payments.
• Ability to work effectively in a fast-paced statutory environment.
• Up-to-date references covering a minimum of three years.
