What you will do:

Undertake holistic occupational therapy assessments for adults with functional, physical, or cognitive impairments within community and home settings.
Recommend and organise appropriate equipment, assistive technology, and adaptations, both major and minor, to foster independence and mitigate risks.
Complete comprehensive moving and handling assessments, offering guidance to carers, families, and support providers to ensure safe practices.
Develop personalised support plans crafted in line with the Care Act 2014, emphasising prevention, reablement, and strengths-focused interventions.

Person Specification:

Qualifications:
- You will need a Degree or Diploma in Occupational Therapy with evidence of continued professional development geared towards adult social care practice.
Registration and Compliance:
- Must be registered with the Health and Care Professions Council (HCPC) and possess strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding adults procedures.
- An understanding of Disabled Facilities Grants (DFG), moving and handling legislation, and strengths-based practice frameworks is crucial.
