What you will do:

• Facilitate access to appropriate social prescribing and wellbeing services, including community groups and voluntary organisations.

• Support individuals to independently purchase goods and services as part of their care and support plans.

• Provide specialist advice, training, and professional support to Occupational Therapy Assistants and wider staff teams.

• Manage and oversee complex cases, offering strategic guidance and mentoring to fellow Occupational Therapists.

• Support See and Solve community staff, ensuring effective deployment of Occupational Therapy resources.

• Contribute to or lead case management processes, including safeguarding reviews and case conferences.

• Work closely with housing partners and surveyors to plan and review home adaptations, completing joint assessments where required.

• Maintain continuous professional development in line with HCPC standards and the RCOT career framework, including leadership and research activities.

Requirements:

• HCPC registration is essential.

• Minimum of 2 years’ post-qualification experience as an Occupational Therapist, ideally within adult social care.

• A valid driving licence is desirable to support community-based assessments and visits across Hull.
