What you will do:

Facilitate access to appropriate social prescribing and wellbeing services, including community groups and voluntary organisations.

Support individuals to independently purchase goods and services as part of their care and support plans.

Provide specialist advice, training, and professional support to Occupational Therapy Assistants and wider staff teams.

Manage and oversee complex cases, offering strategic guidance and mentoring to fellow Occupational Therapists.

Support See and Solve community staff, ensuring effective deployment of Occupational Therapy resources.

Contribute to or lead case management processes, including safeguarding reviews and case conferences.

Work closely with housing partners and surveyors to plan and review home adaptations, completing joint assessments where required.

Maintain continuous professional development in line with HCPC standards and the RCOT career framework, including leadership and research activities.
Requirements:

HCPC registration is essential.

Minimum of 2 years’ post-qualification experience as an Occupational Therapist, ideally within adult social care.

A valid driving licence is desirable to support community-based assessments and visits across Hull.
