What you will do

• Complete Care Act assessments and deliver appropriate interventions for adults, families, and carers.

• Design, implement, and review care packages as part of comprehensive care planning.

• Collaborate with partner agencies to coordinate services and ensure effective outcomes.

• Assess and respond to adults’ mental health needs in line with departmental procedures.

• Maintain accurate case records and contribute to management reporting and statistical returns.

• Deliver training and consultancy support to colleagues and partner organisations where required.

• Represent the department at specialist multi-agency forums locally and regionally.

• Deputise for the Team Leader when necessary, providing leadership support.

• Carry out additional duties consistent with the nature and responsibilities of the role.

About you

• Qualified Social Worker registered with Social Work England.

• Experience working within adult social care settings.

• Strong knowledge of the Care Act 2014, safeguarding procedures, and mental health legislation.

• Confident managing complex caseloads and working both independently and within a team.

• Committed to maintaining confidentiality in line with the Data Protection Act and adhering to Health and Safety legislation.
