What you will do:
- Conduct comprehensive Care Act assessments, reviews, and care planning for adults with mental health needs.
- Facilitate hospital discharges from mental health hospitals, ensuring all necessary care arrangements are in place.
- Collaborate effectively with multidisciplinary teams including attending ward rounds and liaising with Housing and brokerage.
- Manage complex cases under pressure, prioritising effectively in this fast-paced environment.
- Engage in Court of Protection applications and Mental Health Tribunals as required.
- Carry out Safeguarding Enquiries and duty work to protect vulnerable adults.
- Travel within and out of the borough using your own transport to deliver services efficiently.
This role is for a skilled Mental Health Social Worker working in the local authority mental health social care team. With a keen focus on hospital discharges, you will utilise your expertise in adults' mental health to make a significant impact on the lives of individuals transitioning from hospital settings.
Requirements:
- Registered with Social Work England.
- Experienced in managing complex adult mental health cases.
- Experience working in mental health within a Local Authority setting, including statutory Care Act tasks.
- Proficient in working with multidisciplinary teams and conducting statutory duties such as safeguarding and care planning.
- A driver with access to own transport.
