Working Pattern: Monday to Friday, 09:00 – 17:00

Pay Rate: £21.65 PAYE or £27.49 Umbrella per hour

Seven Resourcing are currently recruiting for an experienced Occupational Therapist to join an Adult Social Care service in Gateshead. This is an excellent opportunity for a qualified Occupational Therapist with experience in community-based Adult Social Care, rehabilitation, equipment provision, and functional assessments.

The successful Occupational Therapist will support adults within the community to maximise independence, improve safety within the home, and reduce risks through effective Occupational Therapy intervention and person-centred care planning.

Job Purpose
To provide high-quality Occupational Therapy assessment and intervention within Adult Social Care, supporting individuals with varying physical, functional, and environmental needs through rehabilitation, equipment provision, and housing adaptation recommendations.

Key Responsibilities

• Undertake Occupational Therapy assessments within community and home settings
• Assess service users’ functional abilities and independence needs
• Recommend and arrange appropriate equipment, aids, and adaptations
• Complete moving and handling assessments and risk assessments
• Support hospital discharge prevention and community independence planning
• Develop person-centred care and rehabilitation plans
• Work collaboratively with Social Workers, health professionals, carers, and housing teams
• Maintain accurate case records, reports, and assessment documentation
• Promote independence, wellbeing, and safe living environments
• Ensure compliance with relevant legislation, policies, and professional standards

Candidate Requirements

• Qualified Occupational Therapist with HCPC registration
• Experience within Adult Social Care or Community Occupational Therapy services
• Strong knowledge of equipment provision and adaptations
• Experience completing functional and risk assessments
• Excellent communication and case management skills
• Ability to work independently and manage a varied caseload
• Strong understanding of safeguarding and person-centred practice
• Experience working within multidisciplinary teams
• Full UK driving licence desirable due to community-based working
