What you will do:
- Conduct comprehensive mental health and risk assessments using the SPA triage process to ensure individuals receive the correct support.
- Provide clinical advice, health promotion, and wellbeing support, encouraging self-management among service users.
- Determine the most suitable care pathways, making referrals to primary or secondary care, specialist teams, or community services.
- Coordinate with multidisciplinary teams to maintain effective communication and ensure seamless care delivery.
- Support the provision of safe, high-quality, recovery-focused care in adherence with NMC standards.
- Collaborate closely with the SPA Team Leader and Clinical Expert to ensure clinical excellence and service quality.
- Maintain in-depth knowledge of local services, community resources, and referral pathways to facilitate prompt access to care.
