What you will do

• Play a key role in the screening of incoming referrals identified by Triagers as relevant for an OT, ensuring timely and appropriate responses to individuals’ needs.
• Undertake and complete initial assessment of referrals using a proportionate approach by gathering information, identifying risk, and determining the most suitable pathway or level of intervention.
• Support efficient service flow by prioritising cases and facilitating early, proportionate responses.
• Provide advice, signposting, and early intervention to individuals with a focus on prevention, reduction, and delay of further care needs.
• Work in a fast-paced environment, delivering high-quality care and support to adults in the community.

Responsibilities include

• Hold a professional qualification as an Occupational Therapist, with HCPC registration.
• Demonstrate at least 2-3 years of experience working as a Band 6 OT within the physical field of health or adult social care.
