Workdays: Monday, Thursday, and Friday (Tuesday also possible but not essential)

What you will do:
- Perform thorough medicines reconciliation, ensuring each patient receives optimal pharmaceutical care.
- Conduct clinical reviews and manage discharge medications, maintaining high standards throughout the inpatient and discharge process.
- Provide expert medicines advice to both patients and healthcare professionals, promoting safe medication practices.
- Contribute to audits, data collection, and attend daily huddles and meetings, supporting overall medicines optimisation.
- Offer minimal dispensary cover as required, demonstrating flexibility and adaptability.
Requirements of the Pharmacist:
- GPHC registration is essential.
- Previous experience in a UK NHS Pharmacy setting.
- NVQ or equivalent qualification as a Pharmacist.
- Confidence and proficiency in managing physical health medicines and long-term conditions.
- Ability to collaborate effectively within a multidisciplinary team.
- Access to a valid NHS smartcard.
