What you will do:
- Assess, develop, implement, and evaluate care plans, ensuring high standards of evidence-based practice.
- Work autonomously, recognising changes in patients' presentations and escalating to senior staff when necessary.
- Guide and direct nursing staff and multi-disciplinary team members in delivering comprehensive care.
- Support patients in their daily living activities and promote health education within support networks.
- Build therapeutic relationships, using adaptable communication skills to meet the needs of diverse situations.
- Document care accurately and maintain clear records in line with agreed policies.
- Promote the highest standards of practice as a professional role model and mentor for junior staff.
