What you will do:

• Complete Occupational Therapy assessments within community settings.
• Undertake functional assessments in service users' own homes.
• Conduct moving and handling assessments, and develop corresponding plans.
• Perform mental capacity assessments.
• Evaluate and recommend seating solutions tailored to individual needs.
• Arrange for the provision of necessary equipment.
• Carry out major and minor adaptations assessments.
• Provide recommendations under Disabled Facilities Grant processes.
• Support individuals to remain safely at home and maximise their independence.
• Maintain accurate records, reports, and case documentation.

Requirements:

• Degree or Diploma in Occupational Therapy.
• HCPC registration.
• Experience in Community Occupational Therapy.
• Proficiency in completing functional and mental capacity assessments.
• Skilled in moving and handling assessments and planning.
• Competence in recommending and providing equipment solutions.
• Experience with adaptations and the Disabled Facilities Grant processes.
• Ability to work independently within community settings.
• Strong assessment and report-writing skills.

Desirable Requirements:

• Experience in Adult Social Care.
• Background in working within Local Authority services.
• Familiarity with hybrid working practices.
