What you will do:

• Provide specialist physiotherapy assessments and interventions within acute medicine wards.

• Assess, treat, and manage acute inpatients to optimise recovery, mobility, and functional independence.

• Support discharge planning activities, ensuring patients are safely and appropriately discharged from hospital.

• Work closely with multidisciplinary colleagues to deliver coordinated, patient-centred care.

• Manage and prioritise a varied caseload effectively, ensuring timely interventions.

• Facilitate patient flow through proactive rehabilitation and discharge planning.

• Complete comprehensive assessments and develop tailored treatment plans to meet individual patient needs.

• Provide education and advice to patients, families, and carers to promote ongoing recovery and self-management.

• Maintain accurate and timely clinical records in accordance with professional standards and local policies.

• Participate in multidisciplinary meetings and contribute to service development and quality improvement initiatives where appropriate.

Requirements:

• Degree or equivalent qualification in Physiotherapy.

• Current HCPC registration.

• Experience working within an acute inpatient setting.

• Proven discharge planning experience.

• Minimum of 6 months' recent UK NHS experience.

• Strong assessment, rehabilitation, and clinical reasoning skills.

• Excellent communication and multidisciplinary working abilities.

• Ability to work independently and manage a busy caseload effectively.
