What you will do:

• Provide high-quality Occupational Therapy assessments and interventions within an acute medicine ward setting.

• Assess and treat acute inpatients, supporting their rehabilitation, recovery, and functional independence.

• Facilitate effective discharge planning to promote safe and timely patient discharges.

• Work collaboratively with multidisciplinary teams to deliver coordinated, patient-centred care.

• Manage and prioritise a varied patient caseload efficiently and effectively.

• Assess equipment, environmental, and support needs to enable safe discharge and prevent readmission.

• Develop person-centred treatment plans tailored to individual patient goals and needs.

• Provide advice, education, and support to patients, families, and carers.

• Maintain accurate and comprehensive clinical records in accordance with professional and organisational standards.

• Contribute to service improvement initiatives and the ongoing development of Occupational Therapy services.

Requirements:

• Degree or equivalent qualification in Occupational Therapy.

• Current HCPC registration.

• Previous experience working within acute inpatient settings.

• Experience with discharge planning and hospital flow processes.

• Minimum of six months' recent UK NHS experience.

• Strong assessment, rehabilitation, and clinical reasoning skills.

• Excellent communication and multidisciplinary team-working abilities.

• Ability to manage a busy and varied caseload effectively.
