What you will do:

• Undertake comprehensive physiotherapy assessments for patients referred through the Discharge to Assess pathway within their home environment.
• Develop, implement, and review individualised treatment and rehabilitation plans to maximise functional independence and wellbeing.
• Assess mobility, falls risk, environmental factors, and equipment requirements, arranging provision where appropriate.
• Work autonomously while managing a complex caseload of patients with frailty, long-term conditions, and multiple co-morbidities.
• Collaborate effectively with multi-disciplinary team colleagues, including Occupational Therapists, Nurses, Social Workers, GPs, and community services.

Person Specification:

Qualifications:

• Degree or Diploma in Physiotherapy recognised within the UK, with current registration with the Health and Care Professions Council (HCPC).
• Evidence of relevant post-registration Continuing Professional Development (CPD) is essential.
• Membership of the Chartered Society of Physiotherapy is desirable.

Registration & Compliance:

• Must be an HCPC Registered Physiotherapist.

Experience:

• Prior experience as a Band 5 or Band 6 physiotherapist within acute, rehabilitation, frailty, or community services is required.
• Experience in discharge planning and supporting patients with complex health needs is essential.
