What you will do:
- Perform comprehensive assessments to support discharge planning and provide guidance tailored to frailty, medical, and acute settings.

To be successful in this role, you must have HCPC registration and possess experience working within acute medical environments or care of the elderly settings. A valid driver's licence is also necessary as you'll need to travel within the community.
