What you will do:

• Encourage and support service users to achieve independence in daily living activities such as personal care, medication management, cooking, cleaning, shopping, and managing finances.

• Assist service users in accessing the community, promoting participation in various social and recreational activities.

• Utilise the Active Support Model to engage and empower individuals, fostering a sense of ownership in their routines.

• Collaborate effectively with colleagues and act as a key worker to develop and implement individual care plans.

• Maintain accurate documentation and adhere to service procedures and quality standards.

• Drive service vehicles when required, ensuring a flexible and adaptable approach to meeting the needs of the service users.
