An exciting opportunity has arisen for a Band 6 Occupational Therapist to join the Community Rehabilitation Team in Basildon. This full-time locum role offers up to £27 per hour for an initial 3-month contract. You will play a vital role in delivering rehabilitation services to vulnerable patients within the community, supporting individuals to regain independence and improve their quality of life following discharge.

What you will do:
- Conduct comprehensive occupational therapy assessments within community settings, including care homes and patients’ own homes.
- Identify functional needs and establish personalised rehabilitation goals for patients on the Discharge to Assess pathway.
- Develop, implement, and review effective rehabilitation and care plans.
- Work collaboratively with multidisciplinary teams to ensure coordinated patient care.
- Monitor patient progress and adapt interventions to achieve the best possible outcomes.
- Provide advice, guidance, and support to patients and families to encourage engagement in rehabilitation programmes.
- Maintain accurate and up-to-date clinical documentation in line with professional standards.
Requirements:
- HCPC registered Occupational Therapist.
- Previous community rehabilitation experience is desirable.
- Ability to work independently within community settings.
- Full UK driving licence and access to transport required for community visits.
