Join an exciting opportunity as a Band 6 Community Rehabilitation Physiotherapist in the heart of Basildon. This rewarding locum role offers up to £27 per hour for a 3-month full-time contract. Working four shifts per week from 08:00 to 18:00, Monday to Friday, you will play a key role in supporting patients through community-based rehabilitation and the Discharge to Assess pathway.

What you will do:

Conduct comprehensive physiotherapy assessments within community settings, including care homes and patients’ residences.
Identify patients’ functional needs and establish achievable rehabilitation goals under the Discharge to Assess pathway.
Develop, implement, and review personalised rehabilitation and treatment plans.
Work collaboratively with multidisciplinary teams to ensure coordinated and effective patient care.
Monitor patient progress and adapt treatment interventions to maximise rehabilitation outcomes.
Provide support, education, and guidance to patients and their families throughout the rehabilitation process.
Maintain accurate and timely clinical records in accordance with professional standards.

Requirements:

HCPC registered Physiotherapist.
Previous community rehabilitation experience is desirable.
Ability to work independently within community settings.
Full UK driving licence required due to travel within the community.
