What you will do

  • Undertake comprehensive mental health assessments and risk evaluations, crafting personalised care plans for adults referred to secondary mental health services.
  • Manage a defined caseload of individuals with complex mental health needs under the Care Programme Approach (CPA).
  • Deliver evidence-based therapeutic interventions and recovery-focused support directly in the community.
  • Monitor both mental and physical health needs, ensuring prompt interventions and escalating care where necessary.
  • Engage in duty work, screening of referrals, and management of CTO cases, alongside depot administrations.
  • Actively participate in ward rounds and multidisciplinary team discussions to enhance service user care.

Skills required / Qualifications needed

  • Registered Mental Health Nurse (RMN) qualification.
  • Current Nursing and Midwifery Council (NMC) registration.
  • Proven track record within adult mental health services, particularly managing complex cases in community settings.
  • Expertise in assessments, CCO or key worker caseload management, and multidisciplinary collaboration.
