What you will do:
- Conduct thorough assessments, planning, implementation, and evaluation of evidence-based care for service users.
- Perform clinical risk assessments and make informed decisions for those contacting the "Out of Hours" service.
- Collaborate on admissions, case conferences, and discharge planning with the multidisciplinary team.
- Coordinate Care Programme Approach assessments and manage a designated caseload.
- Assist in service development by setting quality standards and conducting audits and reviews.
- Fulfill professional responsibilities under Safeguarding Vulnerable Adults and Children Legislation.
- Comply with the Mental Health Act 1983's requirements and facilitate carer and service user involvement.
- Establish therapeutic relationships and implement evidence-based interventions.
- Deliver carers’ assessments, education, and support in line with National Service Framework requirements.
- Provide coverage and support for colleagues as needed.
