We are looking for an experienced individual to join our team as a Band 6 Care Coordinator (RGN/RNLD/OT) in the field of Learning Disabilities in Guernsey, UK. What you will do:
- Provide care coordination for an allocated caseload of adults with learning disabilities and/or autism.
- Conduct comprehensive assessments to identify the needs of clients and their carers.
- Manage a complex caseload, collaborating with professionals across various disciplines to deliver coordinated care.
- Act as a lead professional within the Community Learning Disability Team, ensuring streamlined care management for clients with complex needs.
- Establish and maintain positive relationships with service users, families, carers, and other professionals.
- Prepare reports and documentation relevant to care coordination and the function of the Community Learning Disability Team.
- Collect statistical information related to care for contribution to annual reporting.
- Maintain accurate and up-to-date case records in line with professional standards.
- Adhere to legislation, policies, and procedures, contributing to policy development.
- Provide guidance and support to colleagues, contribute to multi-agency training, and mentor less experienced staff.
- Promote the rights and independence of service users through active involvement and empowerment.
