What you will do:

• Undertake strengths-based Care Act 2014 assessments and reviews.

• Develop personalised care and support plans that promote independence and meet individual goals.

• Manage safeguarding enquiries and interventions in line with Care Act responsibilities and local procedures.

• Apply the principles of the Mental Capacity Act 2005 in assessments, care planning, and decision-making.

• Work collaboratively with multidisciplinary teams, health professionals, and Continuing Healthcare (CHC) services.

• Advocate on behalf of adults, ensuring their rights, wishes, and outcomes remain central to all interventions.

• Support adults and carers through complex situations, providing professional guidance and practical solutions.

• Maintain accurate, timely, and comprehensive case records in line with legislative and organisational requirements.

• Manage a varied caseload while ensuring statutory responsibilities and performance expectations are met.

• Promote strengths-based, person-centred, and outcome-focused practice throughout all aspects of service delivery.
