What you will do:

• Conduct comprehensive mental health assessments, including risk assessments, within a dynamic multi-disciplinary assessment team. Collaborate closely with the Consultant Psychiatrist to ensure a holistic team approach.
• Efficiently refer patients to appropriate clinicians within the service or to external services/agencies when necessary to ensure seamless patient care.
• Deliver a rapid, responsive service to evaluate individuals' needs for access to mental health services, ensuring timely intervention.
• Foster effective communication between GPs, IAPT, and secondary mental health services to maintain the highest standards of patient care.
• Provide mentorship and support to colleagues, contributing to a culture of learning and development within the team.
