What you will do:

- Conduct occupational therapy assessments within an acute medical setting.
- Provide and manage the provision of necessary equipment to ensure patient needs are met.
- Plan and coordinate discharge for patients to ensure seamless transitions back home or to other care settings.
- Regularly attend board rounds to collaborate with the multidisciplinary team, sharing insights and updates on patient progress.

Requirements:

- Must be registered with the Health and Care Professions Council (HCPC).
- Ability and willingness to drive as part of the role, facilitating patient and site visits as needed.
