What you will do:

  • Work across specialist ward environments, including elderly care wards, medical wards, and older adult mental health services.

  • Undertake medicines reconciliation activities to ensure safe and effective medication management.

  • Complete patients' own drugs assessments and support medicines optimisation initiatives.

  • Manage medicine discharge processes, helping to facilitate safe and timely patient discharges.

  • Supervise and support Pharmacy Assistants and other support staff where appropriate.

  • Undertake Accredited Checking Pharmacy Technician (ACPT) duties if qualified.

  • Provide ward-based dispensing support without routine commitment to hospital dispensary work.

  • Work collaboratively with pharmacists, nurses, medical teams, and other healthcare professionals to deliver high-quality pharmaceutical care.

  • Support the implementation and utilisation of electronic prescribing systems, including BetterMeds, with training provided where required.

  • Maintain accurate records and documentation in line with legal, professional, and organisational requirements.

  • Contribute to patient safety initiatives and quality improvement activities within the pharmacy service.

Person Specification:

  • Current registration with the General Pharmaceutical Council (GPhC).

  • Qualified Pharmacy Technician with experience in hospital or ward-based pharmacy settings.

  • Accredited Checking Pharmacy Technician (ACPT) qualification desirable.

  • Experience undertaking medicines reconciliation and medicines management activities.

  • Full UK driving licence and access to a vehicle due to travel requirements between hospital sites.

  • Enhanced DBS for Adults.

  • Strong communication, organisational, and teamwork skills.

  • Ability to work independently and as part of a multidisciplinary healthcare team.
