This exciting ongoing locum opportunity offers full-time hours of 36 per week, working Monday to Friday from 8:30am to 4:30pm. What you will do:

  • Undertake Care Act 2014 assessments, reviews, and support planning to meet individual needs and promote independence.

  • Complete safeguarding enquiries and manage complex adult social care casework.

  • Apply strengths-based approaches to empower adults and support their wellbeing and independence.

  • Develop person-centred care and support plans in partnership with adults, families, carers, and relevant professionals.

  • Work collaboratively with health colleagues and partner agencies within an integrated service model.

  • Undertake Mental Capacity Assessments and apply the principles of the Mental Capacity Act 2005 where appropriate.

  • Manage a diverse caseload effectively while ensuring compliance with statutory duties and timescales.

  • Maintain accurate, comprehensive, and timely case records, assessments, and reports.

  • Support adults with learning disabilities, complex needs, and those transitioning from children's to adult services.

  • Contribute to multidisciplinary meetings, reviews, and care planning discussions.

  • Promote positive outcomes through effective risk management, advocacy, and person-centred interventions.
