What you will do:

• Conduct comprehensive Occupational Therapy assessments for individuals living in the community.

• Identify risks and recommend appropriate interventions to support safe and independent living.

• Assess the need for equipment, adaptations, and environmental modifications to improve functional outcomes.

• Provide advice and guidance to service users, carers, and families regarding safe daily living activities.

• Apply knowledge of moving and handling techniques and promote safe single-handed care approaches.

• Complete Disabled Facilities Grant (DFG) assessments and provide recommendations for major and minor adaptations.

• Undertake seating assessments to improve comfort, posture, mobility, and independence.

• Apply the principles of the Mental Capacity Act when undertaking assessments involving equipment provision and adaptation work.

• Liaise with housing services, contractors, healthcare professionals, and other partner agencies to deliver coordinated support.

• Maintain accurate and timely case records, assessments, and reports in line with professional and organisational standards.

• Travel to various community locations across Solihull as part of your role.

Requirements:

• Degree or equivalent qualification in Occupational Therapy.

• Current HCPC registration.

• Experience within community Occupational Therapy services.

• Knowledge of equipment provision, moving and handling, seating assessments, and home adaptations.

• Experience with Disabled Facilities Grants (DFG) and adaptation processes is desirable.

• Understanding of the Mental Capacity Act and its application in practice.

• Full UK driving licence and access to a vehicle.

• Strong communication, assessment, and multidisciplinary working skills.
