What you will do:

• Facilitate safe, effective, and timely hospital discharges, ensuring smooth transitions from hospital to community care.

• Undertake Care Act 2014 assessments to identify long-term care and support needs.

• Complete strengths-based assessments and develop person-centred support plans.

• Work collaboratively with NHS colleagues, community health services, and partner agencies.

• Conduct risk assessments and implement proportionate plans that promote independence and safety.

• Undertake Mental Capacity Assessments and apply the principles of the Mental Capacity Act 2005 where appropriate.

• Participate in safeguarding enquiries and ensure adults at risk are protected through appropriate interventions.

• Support adults and their families through complex discharge planning processes, providing advice, guidance, and advocacy where required.

• Maintain accurate, timely, and comprehensive case records in accordance with statutory requirements and local policies.

• Manage a busy caseload effectively while meeting statutory duties, performance targets, and agreed timescales.

• Contribute to multidisciplinary meetings and decision-making processes to achieve positive outcomes for service users.
