What you will do:

Facilitate safe, effective, and timely hospital discharges, ensuring smooth transitions from hospital to community care.

Undertake Care Act 2014 assessments to identify long-term care and support needs.

Complete strengths-based assessments and develop person-centred support plans.

Work collaboratively with NHS colleagues, community health services, and partner agencies.

Conduct risk assessments and implement proportionate plans that promote independence and safety.

Undertake Mental Capacity Assessments and apply the principles of the Mental Capacity Act 2005 where appropriate.

Participate in safeguarding enquiries and ensure adults at risk are protected through appropriate interventions.

Support adults and their families through complex discharge planning processes, providing advice, guidance, and advocacy where required.

Maintain accurate, timely, and comprehensive case records in accordance with statutory requirements and local policies.

Manage a busy caseload effectively while meeting statutory duties, performance targets, and agreed timescales.

Contribute to multidisciplinary meetings and decision-making processes to achieve positive outcomes for service users.
