What you will do:

• Provide practical, emotional, and social support to individuals with learning disabilities, facilitating their independence and engagement in daily activities.
• Assist service users with personal care, medication administration, meal preparation, household tasks, budgeting, and community access, following their care plans.
• Uphold wellbeing, dignity, safeguarding, and human rights while adopting a person-centred and strengths-based approach.
• Act as a Key Worker for designated individuals, aiding in care planning, reviews, health appointments, and achieving person-centred outcomes.
• Keep accurate care records, incident reports, and documentation in accordance with organisational procedures and regulatory standards.

Person Specification:

Qualifications:

• NVQ Level 3 Diploma in Health and Social Care, Care Certificate, or the motivation to pursue an equivalent qualification.
• Proficient literacy and numeracy skills for maintaining care records and reports.
• Full driving licence preferred, with a willingness to drive service vehicles as needed.

Experience:

• Previous experience supporting adults with learning disabilities, autism, mental health needs, or complex care requirements.
• Background in residential, supported living, community care, or social care settings.
• Proven success in promoting independence, social inclusion, and positive outcomes for vulnerable adults.
• Experience collaborating effectively within a multidisciplinary team and maintaining professional care records.
