What you will do:

  • Conduct detailed Social Care Assessments to identify eligible needs and outcomes.
  • Develop and implement Care Planning processes to support individuals effectively.
  • Collaborate with carers, completing Joint Carers Assessments as part of a holistic approach.
  • Serve as Enquiry Practitioner in Safeguarding Adult cases, ensuring the wellbeing and protection of vulnerable adults.
  • Apply the Mental Capacity Act in making Best Interest Decisions, respecting the autonomy and rights of those you work with.

Qualifications and Requirements:

  • Relevant qualification in Social Work such as CQSW, DIPSW, CSS, or a Degree in Social Work.
  • Registration with Social Work England.
  • Full UK driving licence and access to a vehicle are essential for visiting clients and making assessments.
