What you will do:

• Manage a caseload, working collaboratively with colleagues from Norfolk and Suffolk Foundation Trust (NSFT) and other partners to ensure timely discharges and reviews in accordance with the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
• Engage closely with NSFT and a broad spectrum of health professionals, making a genuine difference in the fast-paced world of social care.
• Facilitate assessments and develop care plans for older people with complex needs, drawing on your expertise or interest in mental health social care.
• Showcase your ability to work under pressure, demonstrating a high level of organisation and prioritisation skills.
