What you will do:

• Manage a varied community caseload that involves working with complex situations, safeguarding enquiries, and conducting Mental Capacity Act assessments
• Contribute to timely, proportionate assessments and outcome-focused planning to ensure individuals receive the support they need
• Engage in meaningful conversations guided by Community Led Support principles to foster independence and resilience among adults
• Work in collaboration with health and social care partners, providing support that aligns with Care Act responsibilities
• Support early intervention efforts through creative, community-based solutions
• Maintain strong organisational skills and exercise sound professional judgement in a multidisciplinary environment

Bring your expertise to our team if you are registered with Social Work England and have confidence working with adults with physical disabilities and older people. A familiarity with Norfolk County Council systems will be highly advantageous.
