Job Title: Qualified Social Worker – Intermediate Care
Job Location: Akerman Medical Practice, SW9 6AF, with some occasional work at Lambeth Civic Centre, Brixton, SW2 1EG
Salary: £32 hourly
An exciting opportunity has arisen for a Qualified Social Worker to join the Intermediate Care team in Lambeth. This position offers a six-month contract, working full-time on a 35-hour week from Monday to Friday, 9am to 5pm with a one-hour unpaid lunch break. You will be part of the Lambeth Rehab & Reablement Team, contributing to the progress of adults receiving therapy and reablement support.

What you will do:
- Complete strength-based, person-centred assessments and reviews for adults receiving reablement and therapy input.
- Evaluate individual progress against therapy goals and recommend appropriate ongoing care levels.
- Collaborate effectively with Occupational Therapists, Physiotherapists, and other allied health professionals from GSTT.
- Participate in weekly multi-disciplinary team meetings and engage in various development sessions.
- Contribute to data collection projects that are essential for the improvement of integrated care services.
- Undertake Care Act assessments, statutory reviews, and develop/update Care and Support Plans.
- Perform safeguarding work and conduct Mental Capacity Act (MCA) assessments, which include best interest decisions.
- Maintain detailed case records and adhere to legislative requirements and local policies.
- Work in tandem with community services, therapy teams, carers, and families to support rehabilitation efforts.
The successful candidate must:
- Be a Registered Social Worker with Social Work England.
- Have a minimum of two years of post-qualifying experience in services such as Intermediate Care, Reablement, and integrated health and social care.
- Possess excellent assessment and analytical skills, ready to excel in a fast-paced, multidisciplinary environment.
- Provide reliable references from recent roles in social work.
