What you will do:

Manage a caseload as a Care Coordinator under the Care Programme Approach (CPA).
Undertake comprehensive mental health and risk assessments to develop tailored care plans.
Develop and review personalised care strategies centred around each patient’s needs.
Deliver therapeutic interventions, promoting a recovery-focused approach.
Administer and monitor medications to support patient wellbeing.
Liaise with multidisciplinary teams to ensure seamless handovers and patient transitions.
Respond to both urgent and routine queries as part of duty cover, maintaining a responsive service.

Person Specification:

Qualifications:

Registered Mental Health Nurse (RMN) with valid NMC registration.
Enhanced DBS and right to work in the UK.

Key Skills:

Strong expertise in mental health assessment and risk management.
Excellent communication and therapeutic engagement skills.
Ability to manage complex caseloads and maintain accurate documentation.
Experience working within community mental health settings and CPA processes.
Effective collaboration within multidisciplinary teams.

Experience:

Proven experience as an RMN within community or mental health services.
Experience managing complex mental health conditions within a CMHT or similar setting.
Strong track record of multidisciplinary and multi-agency working.
